MVP Spawning
- OLD: Dynamic spawn count based on players in map (1-10 MVPs)
- NEW: Fixed progression: 1-6 MVPs based on wave/difficulty

- REMOVED: Dynamic HP scaling based on party size
- MVPs now use default HP values
